Provides people residing in residential aged care homes with additional support to manage their health care goals.

Our Nurse Practitioners work with residents and their family to provide greater access to primary care and assist residents in meeting their health care goals. The service is provided in addition to the current healthcare team and is not a replacement for any existing services.

What is a Nurse Practitioner?

A Nurse Practitioner is authorised to function autonomously and collaboratively in an advanced and extended clinical role.

The Nurse Practitioner is patient-centred and will work collaboratively with residents and their family as well as their General Practitioner, Residential Care staff, specialist and allied health care providers to provide timely and holistic healthcare.

What can Nurse Practitioners do?

Nurse Practitioners are able to:

  • Perform advanced clinical assessments and manage conditions
  • Order and review diagnostic tests, such as blood and radiology tests
  • Diagnose and treat health conditions
  • Initiate referrals to other health care providers
  • Prescribe and review medications
  • Provide holistic health care support
  • Monitor and support long-term health needs
  • Support Advance Care planning discussions
