GP Referrals

GPs can discuss the services with their patients. Patients can be encouraged to self-refer or the treating GP can refer directly by sending a comprehensive referral letter and/or a completed Mental Health Treatment Plan to HPC Psychology Services on the confidential fax number 4929 7072.

*Please note, patients do not require a Mental Health Treatment Plan to access the Drug &Alcohol Program.

Referrals are reviewed daily by an experienced clinician, to ensure that our service is appropriate to meet the patient’s needs. In cases where we are unable to meet the patient’s needs, we will contact you to suggest a more appropriate service. To assist efficient processing of referrals, please provide comprehensive details of the assessment and treatment plan

If you would like to speak to someone about a referral or are wondering whether a referral may be appropriate, please contact the Referral Management Team on (02) 4926 0529 (available every afternoon).

Self Referrals

You can self-refer by contacting HPC Psychology Services on 4926 0529 and ask to speak to one of our specialist drug and alcohol Psychologists, who will talk to you more about the difficulties you are experiencing. Our staff are available each afternoon and if you require more support than we are able to offer, we may transfer you to other appropriate Drug & Alcohol services in the region for further care.
