Self referring to Hunter Primary Care Services
At Hunter Primary Care our focus is on you. We support people to live a healthy life.
Our team of health professionals have experience working with a network of Hunter health professionals to deliver patient-centred care that supports you.
This means we:
- Communicate and coordinate patient care to relevant providers
- Deliver tailored care to suit your needs and goals
- Support you to understand and learn how you can manage your health
Self-referral is available to our services below. If you would like to access any of our other services not listed below, please speak to your GP for a referral.
headspace Newcastle
If you are aged 12 - 25, headspace Newcastle can provide you with information and support on a wide range of issues. For further information and to discuss whether a referral may be appropriate, contact the headspace Newcastle team on (02) 4929 4201 or email headspacenewcastle@hunterprimarycare.com.au
NDIS Support Coordination
If you are an eligible participant with an existing plan and you would like to nominate Hunter Primary Care for Support Coordination, please complete our online referral from or contact the NDIS Intake team on (02) 4925 2259.
Hunter Psychosocial Support Service

Psychology Services
Please speak with your GP about a potential referral to any of our psychology services. Your GP can review your mental health needs and can send a referral to the Hunter Primary Care Access and Referral team to ensure you receive the most appropriate service for you.
Drug and Alcohol Services
You can self refer by contacting Hunter Primary Care Psychology Services on (02) 4925 2259 and ask to speak with one of our specialist drug and alcohol Psychologists. They will talk to you to find out more about the difficulties you are experiencing. Our staff are available each afternoon.
If you need more support than we are able to offer, we may need to transfer you to more appropriate Drug and Alcohol services for further care.
Williamtown Contamination
If you live or work or have lived or worked in the Williamtown area in the last 15 years and have been affected by the PFAS contamination, Hunter Primary Care can provide you with psychological counselling and support.

All health/personal information gathered during the provision of these services will be covered by the usual privacy and confidentiality guidelines. No identifying information will be provided to the PHN or the Commonwealth Department of Health.
