Our team of health professionals have experience working with a network of Hunter health professionals to deliver patient-centred care that supports your clients. This means we:
- Communicate and coordinate patient care to relevant providers
- Deliver tailored care to suit your client's needs and goals
- Support your client to understand and learn how they can manage their health.

Our Aboriginal & Torres Strait Islander Health Worker team can link clients with health and community services, so they feel more connected to community. We accept both self-referral and GP referral to our Aboriginal Health Worker Service.

To refer to our Care Coordination and Supplementary Services (CCSS) please click here to complete our referral form and email it to concierge@hunterprimarycare.com.au We will also need you to provide the following documentation:
- A current GP Management Plan and/or Team Care Arrangement and/or Mental Health Care Plan,
- A current 715 Health Assessment
- Evidence of the patient having one of the following chronic diseases requiring complex care coordination:
- diabetes;
- cardiovascular disease;
- chronic renal disease;
- chronic respiratory disease;
- cancer
- mental health conditions

The Aboriginal and Torres Strait Islander Aftercare Program (Aboriginal Aftercare) supports people who have recently experienced a recent suicide attempt or suicidal crisis. The program provides Aboriginal and Torres Strait Islander people with non-clinical, practical support in the community for up to three months, tailored to individual needs.
People referred to the service are linked with an Aboriginal Support Coordinator who can help connect with the supports that are right for that person.

Clinical Care Coordination provides time limited support (up to 12 months) to adults with complex and enduring mental health and psychosocial needs.
Referral to Hunter Primary Care's Clinical Care Coordination service is available by professional referral only. You can refer via secure clinical messaging ("gp psychology" in the Medical Objects Referral Client online address book) or via SENT referrals.

If you a provider with an NDIS participant requiring Support Coordination, Specialist Support Coordination, Allied Health or Positive Behaviour Support, please Click here to visit our NDIS Services page and complete our online referral form.
You can also email a copy of your participants NDIS Plan and relevant information to NDIS@hunterprimarycare.com.au.
This will provide us with an understanding of the level of supports and services that your patient will need. Our team will then contact your patient (within 2 days of referral) and arrange a meeting (within the next 5 days).

Hunter Primary Care accept GP, allied health professional or nurse practitioner referrals to our Priority Allied Health Service. Please download the appropriate referral form below and return the completed form to info@hunterprimarycare.com.au

If you are a medical professional and would like to refer a patient to our psychology services, please refer using the following MBS items:
- a GP managing the patient under a GP Mental Health Treatment Plan (2700, 2701, 2715 or 2717) or under a Psychiatrist Assessment and Management Plan (291)
OR
- a psychiatrist or pediatrician following the provision of a service and a claim under a specific Medicare item number (for specialist psychiatrists and pediatricians, following a claim for any item within the range (104—109); for consultant physician psychiatrists, following a claim for any item within the range (293—370); and for consultant physician pediatricians, following a claim for any item within the range (110--133).

Referrals are reviewed daily by our experienced Access and Referral Managers to ensure that our service is appropriate to meet your patient’s needs. In cases where we are unable to meet your patient’s needs, we will contact you to suggest a more appropriate service. To assist with the efficient processing of referrals, please provide comprehensive details of the assessment and treatment plan.

The Way Back Support Service works closely with Calvary Mater Newcastle who links people directly with The Way Back following a deliberate self-poisoning event.
