Aboriginal and Torres Strait Islander Aftercare Program
The Aboriginal and Torres Strait Islander Aftercare Program (Aboriginal Aftercare) supports people who have experienced a recent suicide attempt or suicidal crisis. The program provides Aboriginal & Torres Strait Islander people with non-clinical, practical support in the community for up to three months, tailored to individual needs.
People referred to the service are linked with an Aboriginal Support Coordinator who can help connect with the supports that are right for that person.
Examples of support includes:
- Providing support by keeping in touch to have a yarn;
- Developing a safety plan;
- Help to connect with services;
- Helping to build cultural connections
The Aboriginal Health & Torres Strait Islander Aftercare Program is voluntary and available at no cost to participants.
Who is in the team?
The Aboriginal Support Coordinator role is an identified position and works specifically supporting Indigenous people. The program is part of Hunter Primary Care and works closely with other teams including The Way Back Support Service and Yudhilidin.

How to refer
Referrals are accepted from both hospital and community members, and can be made by self-referral and professional referral by your GP or health care provider. GPs can also refer via BPAC SeNT e-referral system.
