Hunter Primary Care’s allied health professionals work with you to give you the best care possible so that you can live an independent and active life.

Our multi-disciplinary team consists of dietitians, exercise physiologists and occupational therapists.

What is allied health?

Allied health professionals use evidence-based practices to prevent, diagnose and treat various conditions and illnesses. Our NDIS allied health services provides therapeutic supports to participants, as well as assessments and reports that inform access and planning decisions for your NDIS plan.

What allied health services does Hunter Primary Care offer?

We are a registered NDIS provider offering the following supports:

  • Dietetics - Our specialist team of dietitians study your nutritional needs and provide dietary supports, such as menu planning and diabetes management, to ensure your nutritional needs are met.
  • Exercise Physiology - Our experienced exercise physiologists create exercise programs to help improve function, movement, reduce pain, prevent future health issues/incapacity, and improve overall health, functioning and quality of life.
  • Occupational Therapy – Our specialised occupational therapists provide personalised, goal-centred support to re-enable you to participate in daily living tasks and recreational activities. We focus on regaining abilities, managing pain, functional capacity and modifying your environment to support your lifestyle.
  • Social Work - Our accredited social workers support NDIS participants in a number of areas such as daily planning and goal setting, navigating and advocacy with systems, support with life stages and transitions, as well as psychological interventions to improve mental health and wellbeing and functional capacity assessments.
  • Level 3 Specialist Support Coordination - Our NDIS team are experts in Support Coordination and can provide Level 3 Specialist Support Coordination to residents in the Hunter region. Click here to find out more.

How do I nominate Hunter Primary Care for allied health services?

Once you choose Hunter Primary Care for allied health services and your referral form has been completed, it can be sent through to the NDIS allied health team. This will provide us with an understanding of the level of supports and services that you will need. Our team will then contact you (within 2 days of referral) and arrange a meeting (within the next 5 days). If you are unable to complete your referral, contact the NDIS team on (02) 4925 2259.

How to refer

If you are an eligible NDIS participant with an existing NDIS plan and you would like to nominate Hunter Primary Care for allied health, contact our NDIS team on (02) 4925 2259 or email NDIS@hunterprimarycare.com.au.
