NDIS Services
As a registered NDIS provider, Hunter Primary Care has been assisting people with their NDIS plans since the scheme was first piloted in the Hunter region in 2013. We are passionate about helping participants to achieve their goals and reach their potential.
To support you and your NDIS participant on their disability and wellbeing journey, Hunter Primary Care will:
- Ensure choice and control by working with you to identify a budget and timeframe
- Work with you to set goals that aim to decrease the need for this high-intensity support to continue
- Address barriers and reduce complexities, by improving access to clinical services through the coordination of supports and services
- Connect your NDIS participant with supports to build their capacity and resilience
Our team have positive and collaborative relationships with the GP's they work with, and value open communication between all parties to ensure the participant is able to live the life they want. They can assist you with identifying patients who are on the NDIS, or provide comprehensive reports, assessments and updates for current NDIS participants. Our team provide a professional and personalised service by spending time with clients in their homes, building capacity and creating real change over time.

Health and Wellbeing Services
- Aboriginal Health and Wellbeing including care coordination from Dietitians, Occupational Therapists, Exercise Physiologists and Registered Nurses, mental health and psychology services, suicide prevention and recovery services
- General Health and Wellbeing including GP Access After Hours, Aged Care Emergency (ACE) and Priority Allied Health services
- Mental Health and Wellbeing including headspace Newcastle, Commonwealth Psychosocial Support (CPS), psychology services, mental health care coordination, and suicide prevention and recovery services.
