HPMI Weekend 2022

Thank you for your interest in Hunter Primary Care health and wellbeing services.

NDIS Services

As a registered NDIS provider, Hunter Primary Care has been assisting people with their NDIS plans since the scheme was first piloted in the Hunter region in 2013. We are passionate about helping participants to achieve their goals and reach their potential.

To support you and your NDIS participant on their disability and wellbeing journey, Hunter Primary Care will:

  • Ensure choice and control by working with you to identify a budget and timeframe
  • Work with you to set goals that aim to decrease the need for this high-intensity support to continue
  • Address barriers and reduce complexities, by improving access to clinical services through the coordination of supports and services
  • Connect your NDIS participant with supports to build their capacity and resilience

Our team have positive and collaborative relationships with the GP's they work with, and value open communication between all parties to ensure the participant is able to live the life they want. They can assist you with identifying patients who are on the NDIS, or provide comprehensive reports, assessments and updates for current NDIS participants. Our team provide a professional and personalised service by spending time with clients in their homes, building capacity and creating real change over time.
