Coordinated Respiratory Support
Linking Muswellbrook residents with comprehensive respiratory care from accredited healthcare professionals.
Experience the benefits of a collaborative approach to healthcare with Hunter Primary Care’s Coordinated Respiratory Support pilot program. This program is fully funded by the Primary Health Network and is available to residents of the Muswellbrook LGA.
The program brings together a mix of local accredited healthcare professionals from varying clinical fields, working together as your comprehensive respiratory care team. You will work with a respiratory nurse and allied health professionals which may include physiotherapists, occupational therapists, dietitians, exercise physiologists and psychologists.
The Coordinated Respiratory Support program is here to help you access the healthcare services you need locally. Our team will work closely with your GP to make sure care is seamless, efficient, and well-communicated. You’ll only need to share your story once, as we’ll keep your GP informed throughout the process.
The program spans across twelve months and includes:
- A comprehensive health assessment by a registered nurse care coordinator to understand your needs and create a tailored care plan
- Consultations with local allied health professionals
- Fortnightly check-ins with your care coordinator to monitor progress and make adjustments as needed
- Collaborative meetings involving you, your GP and your allied health team to review care plans and treatment
- Optional online Shared Health Appointments to enhance understanding of respiratory health with lifestyle medicine alongside others with lived experience
- Specialised clinical reviews at six and nine months to assess your progress.
With a focus on collaboration, technology, and accessible services, the Coordinated Respiratory Support program is designed to empower both you and your healthcare providers to achieve positive health outcomes.
Eligibility
- Individuals with chronic obstructive pulmonary disease (COPD), severe asthma, cystic fibrosis, pulmonary fibrosis, bronchiectasis, or pneumoconiosis.
- Individuals residing in the Muswellbrook LGA with an eligible diagnosis and a GP Management Plan (GPMP), where available.
How to refer
Referral to Hunter Primary Care's CRS is available by professional referral only. Please speak with your GP about a referral.
