The Hunter New England Central Coast PHN (PHN) has recently announced they will move forward with the Australian Government’s transition to a Head to Health Statewide Intake and Assessment Hub & Spoke Model. Click here to view the announcement.
From 1st October 2024, referrals to PRIMA will cease. Instead, referrals will need to be directed to the statewide Intake and Assessment Hub. A local Referral Spoke will then allocate assessed referrals to local mental health services. Hunter Primary Care will coordinate the Referral Spoke for the PHN.
We would like to assure our valued clients and referrers that Hunter Primary Care will continue to provide high quality, person-centred and evidence-based primary mental health services to vulnerable people in our community. Only the referral pathway is changing. The PHN-commissioned services we deliver include:
- Priority Populations, including:
- People at risk of suicide and self-harm
- First Nations people
- Culturally and Linguistically Diverse people
- LGBTQIA+ people
- Older people living alone/isolated
- People in the perinatal period
- Alcohol and Other Drug Use
- Residential Aged Care Services
