At Hunter Primary Care, we deliver a wide range of Aged Care and Allied Health services designed to improve health, independence, and quality of life across all stages of adulthood.
Our aged care programs, such as the Aged Care Emergency (ACE) service and nurse practitioner-led clinical support, help residential care providers deliver safe, timely care and avoid unnecessary hospital transfers. Our Allied Health services support people of all ages with chronic conditions or functional impairments. From diabetes education and respiratory care to dietetics and exercise physiology, our team works in partnership with GPs and other providers to offer coordinated, person-centred care tailored to individual needs.

ACE: Aged Care Emergency Service
The Aged Care Emergency (ACE) service provides triage, consultancy, clinical support, and advice for Residential Aged Care Facility staff, so that care for residents can be delivered in the facility where appropriate, and transfer to hospital is avoided.
