Free support for people living with diabetes
Are you from Singleton, the Upper Hunter, or nearby? Hunter Primary Care invites you to join our award-winning* online lifestyle and support groups, designed for people living with diabetes. This free service is fully funded by the Primary Health Network and connects you with a friendly multidisciplinary team who will support you through:
- Online group sessions
- Personalised advice
- Practical tips and tools to help manage your diabetes and overall health
You’ll learn simple, effective ways to build healthy habits, manage symptoms, and feel more in control of your condition. This program is available to you in your own home by using MS Teams on a smart phone or computer.
What's included?
You’ll have access to online consultations and self-management support from a team of health professionals, including:

Support and guidance for increasing activity levels with our exercise physiologist

Planning outdoor activities and learning benefits of nature as therapy with our occupational therapist

Information about stress and sleep, mindfulness and motivation with our social worker

Practical cooking classes and skills building with our dietitian

Tips to support you get the most out of GP appointments with our registered nurse

Practical support to keep blood glucose levels in range with our diabetes educator

Connecting with people who share similar experiences and concerns about health and wellbeing

One-on-one support and follow up to reach your goals

Referral to this service is available by health professional referral only. Please speak with your GP about a referral.
