Join our award winning lifestyle medicine community groups

If you're from Singleton and living with diabetes, Hunter Primary Care invites you to join our award winning* lifestyle medicine community groups. This service is fully funded by the Primary Health Network for people with diabetes and those at risk of diabetes and involves online group support and in person group sessions.

You will be provided with consultations and self-management support by our multidisciplinary team to support you to learn new skills to better manage your health and chronic conditions. This includes:

Exercise

 

Support and guidance for increasing activity levels with our exercise physiologist

WalkingMan

 

Planning outdoor activities and learning benefits of nature as therapy with our occupational therapist

Bed

 

Information about stress and sleep, mindfulness and motivation with our social worker

Cooking

 

Practical cooking classes and skills building with our dietitian

Nurse_GP

 

Tips to support you get the most out of GP appointments with our registered nurse

Diabetes

 

Practical support to keep blood glucose levels in range with our diabetes educator

Connection

 

Connecting with people who share similar experiences and concerns about health and wellbeing

Helping Hand

 

One-on-one support and follow up to reach your goals

We aim to build your confidence and strengthen community connections. This program is available to you in your own home by using MS Teams on a smart phone or computer. Our social outreach worker will assist you to connect with the group.

How to refer

Referrals to our Multidisciplinary Lifestyle Medicine Community Groups are available by self-referral by clicking here to complete a referral form, or a professional referral by your GP or health care provider.
