Hunter Primary Care is pleased to announce our new Coordinated Respiratory Support pilot program.  This program is fully funded by the Primary Health Network and is available to residents of the Muswellbrook LGA.

The program brings together a mix of local accredited healthcare professionals from varying clinical fields, working together as a comprehensive respiratory care team.  The team may include a respiratory nurse, and allied health professionals such as physiotherapists, occupational therapists, dietitians, exercise physiologists and psychologists.

The program integrates accredited healthcare professionals into a cohesive multidisciplinary team, saving GPs time by providing comprehensive, streamlined support, reducing follow-up workload and ensuring they stay updated with minimal effort.

The program spans across twelve months and includes:
• A comprehensive health assessment by a registered nurse care coordinator
• Consultations with local allied health professionals
• Fortnightly check-ins
• Collaborative meetings with the client, their GP and their allied health team
• Online group Shared Health Appointments to foster understanding of respiratory health and lifestyle medicine
• Specialised clinical reviews

Referrals can be made through a GP.
