A group program exploring strengths, mindfulness, nature therapy and routine. 

Hunter Primary Care offers a strengths-based, holistic group program designed to help adults build emotional resilience, establish supportive routines and connect more deeply with themselves and the natural world. 

Delivered by accredited social workers, The Grounded Path runs over 8 in-person sessions, 60 minutes each, in person at our Warabrook office. This program gives participants the opportunity to: 

  • Build practical tools and strategies to support daily living 
  • Strengthen self-awareness, resilience and emotional balance 
  • Experience the therapeutic benefits of nature 
  • Learn mindfulness and relaxation techniques 
  • Connect with others in a safe and supportive group setting 
  • Participants are welcome to bring a support person or support worker. 

 What the program covers

Eligibility and funding

  • Must be a current NDIS participant, aged 18+
  • This program is accessed via Capacity Building - Improved Daily Living in your NDIS plan

Costs

  • $77.60 per session, per participant
  • $620.80 for 8 sessions (8 week program), per participant
