How to refer

Information for GPs, Psychiatrists & Paediatricians

If you are a medical professional and would like to refer a patient to our:

Drug and Alcohol Services

Please refer using the following MBS items:

  • a GP managing  the patient under a GP Mental Health Treatment Plan (2700, 2701, 2715 or 2717) or under a Psychiatrist Assessment and Management Plan (291)

OR

  • a psychiatrist or pediatrician following the provision of a service and a claim under a specific Medicare item number (for specialist psychiatrists and pediatricians, following a claim for any item within the range (104—109); for consultant physician psychiatrists, following a claim for any item within the range (293—370); and for consultant physician pediatricians, following a claim for any item within the range (110--133).
Residential Aged Care Services

GP referrals are accepted for residents by sending a brief referral letter to Hunter Primary Care Psychology Services. Please indicate that the service is required in Residential Aged Care and send via our confidential fax number (02) 4929 7072. A Mental Health Care Plan is not required when referring residents to this service.

Nurse initiated referrals may be accepted if the resident’s GP has approved their patient to be treated by a psychologist.

Williamtown Contamination Services

Patients can be referred by a GP using a normal referral letter or Mental Health Treatment Plan (not essential).

Our referral process

Referrals are reviewed daily by our experienced Access and Referral Managers to ensure that our service is appropriate to meet your patient’s needs. In cases where we are unable to meet your patient’s needs, we will contact you to suggest a more appropriate service. To assist with the efficient processing of referrals, please provide comprehensive details of the assessment and treatment plan.

If you would like to speak to someone about a referral or are wondering whether a referral may be appropriate, please contact the Access and Referral Management Team on (02) 4925 2259 (available every afternoon).

All health/personal information gathered during the provision of these services will be covered by the usual privacy and confidentiality guidelines. No identifying information will be provided to the PHN or the Commonwealth Department of Health.

Information for other health professionals

Children, women in the perinatal period and Aboriginal and Torres Strait Islander people can be referred initially to Psychology Services via a provisional referral. These referrals must be supported by a referral from a GP within 2-4 weeks. If you wish to discuss a provisional referral, please contact the Access and Referral Management (ARM) team for a copy of the Provisional Referral Form on (02) 4925 2259.

Referral pathways for rural services

A key focus for Hunter Primary Care is to ensure appropriate access to services across the entire Hunter region. Along with services offered in Newcastle and Lake Macquarie, we also provide services in the Cessnock, Kurri Kurri, Singleton and Medowie areas by employed clinicians.

In the Port Stephens (Nelson Bay), Muswellbrook, and Upper Hunter region, Hunter Primary Care will subcontract mental health clinicians to deliver our psychological services. These providers have significant experience and skills in working in primary mental health care and have a thorough knowledge of the local services in their respective regions.
