Highlights from the report include:

  • Securing long-term funding for GP Access After Hours, ensuring stability and access for communities across the region.
  • Launching Soul Clinic in partnership with Soul Hub, delivering trauma-informed, multidisciplinary care for people experiencing homelessness.
  • Expanding Nurse Practitioner services into regional and rural aged care homes, supported by major research partnerships and new funding.
  • Delivering new models of care across mental health, chronic conditions and respiratory health that improve access and support people to live well.
  • Advancing equity and inclusion, including significant reconciliation initiatives and LGBTQ+ inclusion achievements.

The 2025 Impact Report also reflects our strong commitment to innovation and system improvement, including our involvement in major research projects, collaboration with the Primary Health Network, and leadership in cross-sector partnerships such as the Regional Health Collective.
