Graham has engaged with five Hunter Primary Care services to improve his health and wellbeing.  These services include:
• Aboriginal Health Worker
• Care Coordination and Supplementary services
• Clinical Care Coordination
• Commonwealth Psychosocial Support
• Psychology services

A number of interventions and support have been provided to Graham throughout his time with the organisation, including:
• Accessing defibrillator and cardiovascular treatments
• Support to connect with AMS and GPs, following up referrals and attending appointments
• Support with social housing
• Support with accessing food hampers and vouchers
• Accessing a psychiatry assessment, psychology sessions and psychoeducation
• Assistance with a successful NDIS application
