As a registered NDIS provider, Hunter Primary Care has been supporting people in the Hunter region since the scheme was first introduced in 2013. We understand that navigating the NDIS and finding the right supports can feel complex, which is why we are here to guide you every step of the way.

We provide interdisciplinary, collaborative and holistic care that focuses on your overall health and wellbeing. Our services are culturally safe and trauma-informed, supporting your social and emotional wellbeing in a way that feels respectful and empowering.

Our compassionate team takes the time to get to know you, your goals and your support network. We are person-centred in our approach, supporting you to identify your strengths, build resilience, and maintain choice and control in your care.

With strong local health knowledge and an established network of services, we can help you navigate the health system and connect you with the right supports. We also provide evidence-based therapeutic and capacity building supports, including assessments and reports, to help you achieve your goals and live the life you want.

Level 3 NDIS Support Coordination

If you are an NDIS participant with complex needs requiring specialist support, our NDIS team provides Level 3 Specialist Support Coordination.

Hunter Primary Care’s allied health professionals work with you to give you the best care possible so that you can live an independent and active life.

NDIS Group Programs

Hunter Primary Care offers a range of NDIS group programs designed to build skills, confidence and wellbeing in a supportive, inclusive environment.

NDIS Positive Behaviour Support

As a registered NDIS provider, Hunter Primary Care can provide you with an experienced behaviour support practitioner to assist in understanding why inappropriate or challenging behaviours are being used.

NDIS Psychology

As a registered NDIS provider, Hunter Primary Care can provide therapeutic capacity building supports to eligible NDIS participants by a dedicated team of psychologists, social workers and counsellors.
